Transition School to Work 

The Transition School to Work (TSW)  program focuses on the provision of vocational rehabilitation (VR) services to eligible and potentially eligible high school students with disabilities who are interested in employment after leaving high school. The goal is a smooth, seamless transition from high school to post-high school career development and/or employment.

Germantown Municipal School District applied for, and received the Transition School to Work grant, thus allowing GMSD to hire additional transition staff and increase our community-based instruction and work-based learning sites.

Our Staff and Roles 

Courtney Watkins (Transitional Case Manager) and 
Shelby Smith (Job Readiness Coach)

-Administer and score transition assessments 
-Attend IEP Meetings to discuss transition 
-Collaborate with community and government agencies to ensure students are receiving the services they are eligible for, such as SSI, Employment and Community First Choices, Vocational Rehabilitation, as well as the Office of Students with Disabilities at various colleges and universities. 
-Communicate with current CBI and WBL job site supervisors 
-Attend job sites with students, job carving & job coaching
-Meet with community and business leaders to expand WBL sites. 
-Facilitate Pre-Employment Transition Services 
Facilitate social skills classes, job skill focused 
-Career Exploration
